Seasons Hospice & Palliative Care is seeking an Associate Team Director to join our team of professionals. We make a difference every day to patients and families coping with end of life illness in creating a center of excellence in hospice care.
Overview
The Associate Team Director collaborates with the Team Director (TD) in coordinating all aspects of patient and family care within the scope of practice of profession. Collaborates with the TD/CD to provide direction and support for IDG team members, within scope of practice. Collaborates with Site Leadership to support clinical operations, hospice staff, and to promote provider relations in the community.
Qualifications
Minimum of 1 year experience in hospice. Meets the qualifications of a Seasons Chaplain, Music Therapist, Art Therapist, or Social Worker (advanced certification in hospice preferred). Minimum of 1 year of supervisory or mentoring experience preferred Must have reliable automobile, valid driver's license, and the minimum state required liability automobile insurance.
